Very pleasant countryside walk or bike ride
You can go follow roads quite a long way either east or west along the river out of the centre of Kampong Thom city, although the best access to the river bank is going west - initially on the north side, past the market; but beyond the new bridge (at the western edge of town, about 2 km or so from the now rather delapidated original bridge in the centre), there are paths on either bank. It makes for a pleasant half-day excursion, a lovely glimpse of the Cambodian countryside: lots of fishing villages, glowing green ride paddies and so on - and you might not see another foreigner for hours, which is a rare treat. However, even basic little shops run out of someone's house are few and far between, so you need to take plenty of water with you.

Best to see during rainy season
River swells during the rainy season, you can hire a boat to take you across. Really the best time to visit is during Water Festival, usually in November. Provincial boat races go on throughout the weekend, awesome sight to see all the boats going down.
